    Athletic & Activity Consent Waiver

    Any 7th or 8th grade student that tries out for an athletic team must have a completed form on file in the nurse’s office before tryouts begin.

    Community Service Form

    The office tracks all community service hours completed by students.  This information is used to determine awards given.

    Consent Form

    All students are required to have this form on file in the office.  This form grants students' name &/or picture to be published in the media, grants authorization for access to the district's computer system, and acknowledges that the student has received a copy of the school handbook.

    Eye Examination Form

    This form is required for students who are new to the district and have never attended an Illinois school before.

     Transfer students new to Illinois are required to have a eye examination due within 30 days of enrollment.

    Fee Waiver Application

    Students from a family that is below a certain income level may be eligible to have their registration fees waived.

    Dental Form

    Sixth grade students are required to have a dental examination before May 15 of their sixth grade year.

    Transfer students new to Illinois are required to have a dental examination due within 30 days of enrollment.

    Free / Reduced Lunch Program

    Students from a family that is below a certain income level may be eligible for free or reduced lunches.

    Medication Consent Form

    If a student needs to take medicine during school hours, this form must be completed with the medication information and signed by the doctor.

    Nurse’s office; nurse keeps form on file along with medicine.  Student must go to the nurse to take medicine.

    Physical Form

    Sixth grade students and students new to the district are required to have a complete physical examination including immunizations before the first day of school.

    Transfer students new to Illinois are required to have a physical examination due within 30 days of enrollment.

    Planned Absence Form

    When a student knows in advance that he/she will be absent from school, he/she should take the form to their teachers to get assignments that will be missed.  This form is to be filled out at least two days before the absence begins.
